What is an ingrown toenail?

An ingrown toenail occurs when the edge of the nail curls down and grows into the surrounding skin, most commonly affecting the big toe. It often starts with tenderness, redness and swelling at the side of the nail. If left untreated, pain usually worsens and the risk of infection increases.

What causes and helps prevent ingrown toenails?

Ingrown toenails are commonly caused by:

  • Incorrect toenail trimming
  • Trauma to the nail
  • Poor foot function
  • Incorrectly fitting footwear

A few simple changes help reduce the risk of ingrown toenails, support treatment results, and maintain long-term nail health:

  • Avoid cutting nails too short
  • Cut nails straight across and avoid rounding the edges
  • Wear well-fitting, supportive footwear that doesn't squeeze the toes


Ingrown toenail treatment at Clear Step Podiatry

Most ingrown toenails can be treated without surgery. In more severe or recurring cases, a minor procedure may be recommended as a long-term solution. Toenail surgery is a specialist capability at Clear Step Podiatry: we perform both Partial Nail Avulsion (PNA) and Total Nail Avulsion (TNA) at our clinic, with no hospital visit needed.

1. Conservative ingrown toenail treatment for mild to moderate cases

Conservative ingrown nail removal is a non-surgical option to quickly relieve pain and settle irritation around the nail.

A small section of nail is removed to reduce pressure, and the edge is filed to help prevent regrowth into the skin. The area is then cleaned, and a topical antiseptic may be applied, with oral antibiotics prescribed by a doctor if infection is present.

This provides immediate relief by removing the part of the nail causing the discomfort, and most patients can return to normal daily activities straight away. Our podiatrists may recommend ongoing maintenance, depending on the shape of the toenail, and the severity and duration of the ingrown nail.

2. Ingrown toenail surgery for recurring or severe cases

Partial Nail Avulsion (PNA) is a minor procedure used to treat recurring or severe ingrown toenails.

It removes only the part of the nail causing the problem, relieving pain at the source and preventing it from growing back into the skin.

Your podiatrist will use a local anaesthetic for a pain-free procedure. A chemical treatment called phenolisation is then applied to prevent the nail from becoming ingrown in the future.

Total Nail Avulsion (TNA) removes the entire nail plate, and is recommended when the whole nail is affected, such as severely damaged, thickened or repeatedly ingrowing nails. Like PNA, it's performed in-clinic under local anaesthetic, with phenolisation applied to prevent regrowth where appropriate. Offering both procedures onsite is part of what makes Clear Step a specialist destination for nail care.

Recovery after ingrown toenail surgery is typically 3–6 weeks. Most patients rest for the first 1–2 weeks, avoiding tight shoes, long periods on their feet, and sports or swimming until the toe has healed.

You'll have a few follow-up appointments for dressing changes and a final check-up, where your podiatrist will provide clear aftercare advice to help prevent recurrence.
